A CA 125 test measures the amount of the protein CA 125 (cancer antigen 125) in the blood.

This test may be used to monitor certain cancers during and after treatment. In some situations, the test may be used to look for early signs of ovarian cancer in people with a very high risk of the disease

CA-125 normal range

CA-125 levels in the body are measured by units per millimetre (U/mL).

A CA-125 normal range falls between 0 and 35 U/mL. Levels over 35 U/mL may indicate the presence of cancer or other conditions. Not all patients with a high CA-125 result have cancer.

For women with no ovarian cancer history, a high result usually leads to additional testing. In patients who’ve previously had ovarian cancer, high CA-125 levels may indicate a cancer recurrence.

A CA 125 test isn't accurate enough to use for ovarian cancer screening in general because many conditions can increase the level of this protein.

Conditions that can cause an increase in CA 125 include many that aren't cancerous, such as menstruation and uterine fibroids. Certain cancers may also cause an increased level of CA 125, including ovarian, endometrial, peritoneal and fallopian tube cancers.

If your healthcare provider suspects that you may have ovarian cancer or another type of cancer, may undergo additional tests. Other tests that may be helpful in evaluating these cancers include transvaginal or pelvic ultrasound, serum human epididymis protein 4 (HE4), CT, and MRI. A procedure to remove a sample of cells for testing may be needed to confirm the diagnosis

For normal premenopausal women, the overall upper limit of CA125 should be 50 U/mL. However, if the menstrual status is known, the upper normals should be 62 during menses, 51 for the proliferative phase, and 32 for luteal phase. For postmenopausal women, the CA125 levels should be no more than 35 for those with vaginal bleeding and l20 for those without bleeding. The stratified CA125 values may help provide more specific ovarian cancer screening

These results indicate that vitamin D deficiency results in an increase in the risk of developing ovarian cancer and that vitamin supplements may potentially be an efficient way of preventing cancer.

Regular smoking and caffeine consumption decrease CA125 levels.
ROMA index
The Risk of Ovarian Malignancy Algorithm (ROMA) combines the results of HE4, CA125, and menopausal status into a numerical score. The Risk of Malignancy Index (RMI) is the classic diagnostic tool for preoperative diagnosis of ovarian cancer, which was developed in 1990.
The RMI incorporates three main components, which are menopausal status, the ultrasound score, and the serum CA-125 level.
ROMA SCORE
Premenopausal: <1.14 (low risk)
Postmenopausal: <2.99 (low risk)

ROMA has not been validated for the following groups: women previously treated for malignancy, women currently being treated with chemotherapy, pregnant women, or women younger than age 18.
 
For post-menopausal level is 35 U/mLthis is without bleeding and if the patient has a history of bleeding it should be up to 20U/mL
